The Office of the Children's Guardian monitors the time a child or young person spends in the care of specialised substitute residential care providers. This enables us to identify where additional support may be provided to the family or formalised planning for longer-term placements.

If you are not satisfied with how an entity responded to a reportable allegation, you can make a complaint. 

You must use the forms below when making a notification or report under the Reportable Conduct Scheme.

The Head of Relevant Entity (HRE) is the title under the reportable conduct scheme that describes the most senior person in the organisation.

Organisations that fall under the Reportable Conduct Scheme are called ‘relevant entities’ under the Children's Guardian Act 2019.

Organisations under the Reportable Conduct Scheme must have systems in place for preventing, detecting and dealing with reportable conduct and reportable convictions.

The Residential Care Workers Register commenced on 18 July 2022. Designated agencies must record all residential care workers onto the register before they are able to provide care in a residential setting.

The NSW Carers Register is a centralised database of people who have applied to, or become an authorised carer, to provide statutory or supported out-of-home care in NSW.

When agencies become accredited, there may be conditions placed on them. They also take on reporting obligations under the Reportable Conduct Scheme and the Code of Practice.
